Protected Health Information Notice of Privacy Practices

Theralabs, Inc. is required by law to maintain the privacy of Protected Health Information (PHI), and so provide you with the notice of our legal duties, privacy duties, and privacy practices with respect to PHI. PHI is information that may identify you and that relates to your past, present, or future physical or mental health condition and related health care services. This Notice of Privacy Practices describes how we may use and disclose PHI to carry out treatment, payment, or health care operations and for other specified purposes that are permitted or required by law. We are required to provide this notice to you by the Health Insurance Portability and Accountability Act (HIPAA).

Theralabs, Inc. will not use or disclose your PHI without your written authorization, except as described or otherwise permitted by this notice. We reserve the right to update or change our practices to maintain compliance with local, state, and federal laws, and you can receive such updates or corrections upon request.

Ways We May Use Your PHI:

Treatment: We may use PHI to provide and coordinate the treatment and services you receive,

Payments: We may use your PHI for various payment related functions. We may contact your insurance provider, health care or other indicated payer to determine whether it will pay for equipment, supplies, or services, as well as the amount of you co-pay.

Health Care Operations: We may use your PHI for certain operational, administrative and quality assurance activities.

We are permitted to use or disclose your PHI for the following reasons, but Theralabs, Inc., may never have a reason to make some of these disclosures.

To communicate with individuals involved in your health care or payment of your care. (i.e. family members, or other relatives)

Food and Drug Administrations (FDA): We may disclose information to the FDA, or persons under jurisdiction of the FDA. This would be done in the case of the FDA determining adverse events related to services or equipment received from Theralabs, Inc.

As required by law, we will disclose your PHI when required to do so by federal, state, and local laws.

Judicial and Administrative Proceedings: If you are involved in a lawsuit or dispute, we may disclose your PHI in response to a court order or administrative order.

Notification. We may use or disclose your PHI to notify or assist in notifying a family member, or personal representative, or another person responsible for your care regarding your location and general condition.

Military and Veterans: If you are a member of the armed forces, we may release PHI about you as required by military command authorities.

Other Uses and Disclosures of PHI. We will obtain your written authorization before using or disclosing your PHI for purposes other than those provided for above (or as otherwise permitted or required by law).

Your PHI will be kept securely as required by HIPAA, and your privacy is our priority at Theralabs, Inc.
